Meet your school nurse...                                 

 

My name is Gretchen Young and I will be the nurse for the Sloan Creek Middle School.  I am so happy to join the Lovejoy ISD and look forward to working with you and your children as they continue their secondary education!

 

 

Medication Guidelines   

1.        Prescription and non-prescription medication must be brought to the Wellness Center, all medications are kept with the Nurse with the exception of prescription rescue inhalers that have a Physician written request and authorization for self carry.

2.      Medication must be in the original, properly labeled container.

3.      Medication must have a completed written request form with the parent / Guardian signature.

4.      NO loose pills or unlabeled medication will be given to students under any circumstances.

5.      The Wellness Center does not provide any medications (Tylenol and Ibuprofen is not provided by the school).

6.      When a medication is discontinued or at the end of the school year, a parent is to pick up and sign for the remaining medication.  Medication not picked up will be destroyed.

Wellness Center Orientation

 To help your student have a healthy, successful school year please review the following information regarding the Lovejoy Middle / High School Wellness Center.  A more detailed description of the Wellness Center and general school health guidelines can be found in the Student Handbook.

Illness:

 When should a student stay home from school due to illness?

Fever of 100 or above

Vomiting

Diarrhea

Signs and symptoms of the flu

 

Medication:

A student who must take prescription or non-prescription medication during the school day must bring a written request from his or her parent/guardian along with the medication in its original, properly labeled container. 

 

All  medication is to be stored in the nurse’s office, with the exception of prescribed inhalers for the treatment of asthma.  Prescribed inhalers may be carried by students with asthma provided written authorization from the parent or guardian is given to the school nurse along with a written statement from the student’s physician or licensed health care provider stating that the student is capable of safely self administering prescribed medication.  An asthma action plan is provided for this purpose.  All inhalers must have the prescription label on the medicine.

Immunizations:

 Lovejoy ISD and Texas Immunization Law (Texas Education Code Article 2.09) requires all students attending school to have complete and current immunizations.  In order to remain compliant with this law, your child needs to present proof of having received:

 

DPT (Diphtheria/Pertussis/Tetnus)--Three doses required for all students age 9 or older, with one dose given on or after the 4th birthday.  Four doses for those younger than age 9.

 

TD Booster-- ten years after last dose received on or after the 4th birthday.

 

Polio--  Three doses required with one dose given on or after the 4th birthday.

 

MMR -- (Measles/Mumps/Rubella—One dose required on or after first birthday.

 

MMR-- Booster  Required by 12th birthday, usually given ages 4-6 years.

 

Hepatitis B (HBV)--  Series of three doses are required.

 

Varicella (Chicken Pox)--  Students must either show doctor verification of illness OR have had varicella immunization by their 12th birthday.  Students born after September 1, 1994 must have varicella status recorded.  Children receiving the immunization after age 13 will require a two dose series.

 

 Immunization documentation  must have validation (stamp or signature) by a doctor, clinic, or health department and include the month, day and year vaccines were received.  An immunization record from a previous school is an acceptable form of documentation.

 

If a student cannot be immunized for medical reasons, or an immunization conflicts with a student’s religious beliefs, please consult the Student Handbook for further information.

 

Health Screenings—During the middle school years there will be Hearing, Vision, Scoliosis, and Ancanthosis Nigricans health screenings.  Parents will be contacted only if the student has failed the screening process.

 

Students with a health concern which needs attention while at school should have a health history form filled out and a parent should consult with the school nurse.  Nurse Young will be in the Wellness Center Monday through Friday, please call to schedule a time to meet at 469-742-8308.
